Small Steps Achievable Goals

I wanted to create an assessment sheet that I could you use to set small objectives for the children to reach. It's important that I am setting realistic goals for the children and this assessment allows that. I have divided the sheet into three areas, literacy, numeracy and fine/motor skills. Under each I outline my concern and then the objectives and activities I will use to work towards achieving this.

There is two options in this package. One if the child has concerns in each area or one sheet where there is just one concern.

These sheets are editable so you can type in the boxes or you can write in the boxes.

I have found this works as a great communication resource between parents and the review date allows you to ensure you are on target or need to review it.
